
STEP 1: TAKE A TOUR
If you are looking to enroll your loved one, the first step is to take a tour and see us in action. While an appointment is not required to tour the facility, we do recommend you call ahead to ensure a staff member will be available to meet with you. If someone is not available during your preferred time, we can give you an admission packet that includes information about our Center, and we will take your name and contact information, so a staff member can follow up with you to set a time for a tour.

STEP 2: FILL OUT PAPERWORK
Fill out an application and physician’s form. You can download and print these yourself or pick them up at the Center.
These forms must be turned in before the assessment can be scheduled. These completed forms will also secure your spot on the waiting list if we have one. Send completed form to ucmjadcac@gmail.com
For the Physician’s Form, a visit to your loved one’s doctor may not be necessary. You may be able to have the form completed through fax or email, depending on your physician’s preference and how much time has passed since your loved one’s last appointment. Any physician who has good knowledge of your loved one’s medical history can complete the form.
STEP 3: SCHEDULE AN ASSESSMENT
Paperwork must be completed and submitted for an assessment to be scheduled.
Assessments last around 2 hours and are scheduled with Shevita Williams-Townsend our Operations Manager. During this meeting, the primary caregiver will meet with Shevita to discuss the admission paperwork and level of care, and the potential client will participate in an activity and have lunch.
After the assessment, your loved one can start as soon as the next day (depending on the days of the week they are scheduled to attend).
